YASBIZ Vendor Form

Instructions for Completing This Form


WAIT! If you're a community vendor and don't know what YASBIZ is, and are not in the YASBIZ program, then email us at yourfan@streetsmartventures.com to get the "Community Vendor Registration" form link. Thanks!

This form is intended to be filled out by or with the YASBIZ vendor. It must be completed in addition to your agency’s online registration and payment.


Important Notes for Vendors


  1. Role at the Event

    • Vendors will remain vendors for the day.

    • Vendors will not serve as presenters.

  2. Vendor Success Kit (Required for All Vendors) - Every registered vendor must purchase a Vendor Success Kit, which includes:

    • 1 table at the event

    • 1 boxed lunch and snacks

    • 1 vendor toolkit


      Electricity can be added for a $5 add on per vendor if needed.

  3. Table Sharing or Add-Ons

    • If your group doesn’t need a full table per vendor, let us know in the form.

    • By default, each vendor is entitled to one table.

    • Additional tables may be purchased for $25 each if more than one per vendor is needed.

  4. Group or Umbrella Businesses

    • If your vendors operate under one umbrella business and work together:

      • Use one form for the group. The Advisor should enter their own info in the “Vendor Info” section.

      • A Vendor Success Kit must still be purchased for each vendor in the group.

      • Be sure to list all vendors in the group in the designated section of the form.
